Field Safety Notice issued for One Rehab Illusion mobility scooter instructions for use

BHTA member company One Rehab has issued a Field Safety Notice concerning the updated Instructions for Use supplied with its Illusion mobility scooter. The notice, which has also been published on the Medicines and Healthcare products Regulatory Agency (MHRA) website, is addressed to retailers and end users.

What has been identified?

Following an internal review, One Rehab has produced an updated version of the Instructions for Use supplied with the Illusion mobility scooter. The previous manual should be replaced with the updated version, which includes additional guidance in section 11, ‘Batteries and charging’.

The updated manual advises users to:

• Unplug the charger when charging is complete
• Avoid charging the scooter outdoors or in escape routes
• Install smoke alarms in the charging area
• Inspect batteries regularly for damage, swelling, or corrosion
• Charge during the day to reduce the risk to sleeping occupants
• Never modify or adapt the battery or wiring within the battery pack

What is the potential risk?

One Rehab assesses the risk arising from the previous version of the manual as low. The updated manual contains additional information that the manufacturer considers appropriate to support safe ongoing use of the scooter.

Which scooters are affected?

The Field Safety Notice applies to all serial numbers of the following scooter:

• Product: Illusion (HS380L) car-transportable mobility scooter
• Serial numbers affected: All units
• FSN reference: FSN-2026-001

What actions are required from retailers?

For showroom stock that has not yet been supplied to an end user, retailers should check the Instructions for Use accompanying each scooter and request copies of the updated version where required.

For scooters already supplied, retailers should, where possible, check that users have obtained the updated manual. It can be downloaded from the One Rehab website or the Komfi-Rider website. One Rehab can also provide printed copies by post free of charge.

The manufacturer will contact end users directly where their details are held through its warranty registration records.

Retailers should confirm receipt of the notice using the retailer response instructions included in the Field Safety Notice.

What actions are required from end users?

End users should obtain the updated Instructions for Use, read and follow the revised guidance, and retain the manual for future reference. They should also confirm receipt using the end-user response instructions provided in the notice.

Anyone who is uncertain whether they have the current manual should contact One Rehab or the retailer that supplied the scooter.

Passing on the notice

Recipients should pass the notice to relevant people within their organisation and to any organisation to which affected scooters have been transferred, including sub-dealers, installers, or service agents.

They should maintain awareness of the notice for an appropriate period and report device-related incidents to One Rehab and, where appropriate, the MHRA.
